Step 1: Addressing the Returning Officer
Begin by addressing the form to the Returning Officer. You will need to fill in the name of your district and constituency. For example, if you are from NA-1 in the district of Rawalpindi, you would write “PP-1, NA-1, District Rawalpindi.”

Step 2: Providing Personal Details
In the first section of the form, write your name clearly. If you are a son, daughter, or wife of the mentioned person, include their name as well.

Step 3: Residence and Serial Number
Enter your complete residential address. This should match the address on your National Identity Card (NIC). Additionally, you are required to write the serial number that you have been assigned in the electoral roll.

Step 4: Electoral Information
Specify the part of the electoral roll and the electoral area’s name where you are registered. This information determines the constituency in which you are eligible to vote.

Step 5: Constituency Number
Indicate the number and name of your constituency. This information can be found on your voter registration details or through the Election Commission of Pakistan’s website.
