Covid Vaccination Certificate of Parents of Students and Family Member Age 17 Years

A notification has been issued on 2nd September 2021 in connection with the Covid Vaccination Certificate of Parents of Students and Family Member Age 17 Years and above by the Punjab School Education Department. Further detail of said notification in same is as under;

Provision of Covid Certificate of 100% Vaccination of Students Parents

Reference meeting of CEOs (DEAs) held on 01-09-2021 at QAED under the Chairmanship of Minister of School Education wherein the Chair directed to seek vaccination certificates from the students of their parents and family members having age 17 years & above.

Kindly find attached herewith Proforma and format letter (in Urdu) for collection of data / vaccination certificates from parents. Moreover, the consolidated information as per format, may be furnished to this office through Email (Excel format), for onward submission to the quarter concerned, at the earliest.
